Position Description
FAMILY PARTNER III
JOB DESCRIPTION
Division:Families F.I.R.S.T. Date: September 15, 2009 FLSA Status: Exempt
PURPOSE
Performs all the duties as described in Family Partner Levels I and II as well as provides behavioral health/residential safety/IFP interventions for the family within target ratios assuring fidelity of the Families F.I.R.S.T. program. Performs short term clinical and therapeutic interventions/services to children/youth and families.
ACCOUNTABILITIES

Assures confidentiality of information.
Authority to conduct initial evaluations for services as NCFAS for each child/youth and families.
Authority to conduct initial evaluations and assessments for children/youth and families to include completion of the NCFAS.
Assures case plans, goals and objectives are implemented within service provision.
Includes family and team involvement in the development of the case plan.
Complies with all corporate and contract policies and procedures.
Maintains current status for all training related to program and position.
Discretionary decision making skills within scope of job performance.
Completion of all documentation as dictated by program within identified time frame.
ESSENTIAL FUNCTIONS OF THE POSITION AND PERFORMANCE STANDARDS
Coordinates, prepares, plans and manages cases to assure case plan is being carried out for youth/children and families referred by CFS, reviewing case information, making appropriate contacts, and establishing rapport with the family.
Authority to monitor child/youth and parent/caregiver interactions to ensure the safety of all family members and the community.
Documents events and occurrences during sessions relating to inter-actions between parents, child(ren), or child/guardian.
Transports children safely using approved and appropriate car restraints.
Assists in locating suitable alternative placement to meet a specific child’s needs and to meet policy and program specific guidelines.
Provides outpatient therapy services.
Completes Magellan applications for higher level of care as directed.
Develops, recommends, implements, monitors and evaluates family case plan and makes changes where applicable.
Maintains on-going contact with Families F.I.R.S.T Manger, CFS, and service coordinator regarding each case assigned as deemed appropriate or as requested.
Attends program specific training, and/or meetings as specified by supervisor.
Assists with coverage for other staff in the event of an emergency or other specified need.
Completes all reports in a timely, accurate and efficient manner according to program standards to include drafted court reports.
Attends court when requested to by Families F.I.R.S.T Manager and DHHS/CFS.
Attends family meetings or team staffing when requested to do so.
Elevates session and/or safety concerns immediately to Families F.I.R.S.T Manger, CFS, service coordinator.
Responsible for assuring in-home safety of potential crisis situations in the family unit at varied hours throughout the day or night.
Completes NCFAS for each family served and assists in establishing service plan and other assessment tools as directed and deemed appropriate.
Assists in resolving issues or concerns for the family through provision of clinical and non-clinical services.
Participate in direct service delivery to children/youth and families at the direction of the Family Service Coordinator.
Encourage and facilitate ongoing family engagement.
Ensure that outcomes, timelines, and other expectations with regard to service delivery are attained.
Attend court and testify as needed.
Provide back up to other team members to ensure continuity of service providers.
Performs other tasks as may be assigned.

Internal: Works in harmony with all team members, supervisors, managers and other related personnel.
External: Continual contact with families, CFSS, Service Coordinators, and other stakeholders involved with the child/youth and family.
JOB SPECIFICATIONS AND WORK REQUIREMENTS
Education
Required: Minimum of Master’s degree in human services, counseling or related field and Licensure and/or provisional licensure as a mental Health Practitioner.
Experience
Required: Must be at least 21 years of age with a minimum of three (3) years experience working with children and families.
Desired: Prefer a minimum of five (5) to seven (7) years experience in a field directly related to children and/or families.
Licenses, Certifications, and/or Registrations: Clear Criminal History Background and Adult/Child Abuse checks, as well as all program specific certifications, NCI, First Aid/CPR, Car Restraint, Defensive Driving, clearance from all background checks and drug screens (Trainings offered by Visinet, Inc.). May be working on or have LMHP.
Equipment/Tools/Work Aids: Knowledge of Microsoft word and basic computer use helpful.
Specialized Knowledge and Skills: Spanish, Sign Language, etc. beneficial.
Working Conditions:
Varied climates both indoors and outside in homes, neutral locations or related areas.
Varied working hours – typically late afternoon and early evenings and at least one weekend day.
Various other components such as pets, housekeeping skills, etc.
Significant time spent driving between and/or to sessions.
