COMMUNITY TREATMENT AIDE
JOB DESCRIPTION
Division:Behavioral Health Services Date: September 16, 2009 FLSA Status: Non-Exempt
Maintain a strengths-based approach while working with client(s) and families to assist in the retention of dignity and the development of positive intrinsic qualities. Create a therapeutic milieu within the client’s home environment to promote positive change.
1. Efficiency in performing day to day business duties for Visinet, Inc.
2. Complies with all company policies and procedures.
3. Meets all deadlines as set forth by COO/CFO/CEO.
4. Be available to conduct visits/sessions at least three (3) weekday evenings and six (6) hours on Saturdays or Sundays.
6. Maintains appropriate boundaries and confidentiality with client(s) and families, as well as, with all team Members involved in a case.
7. Maintains an acceptable driving record, reliable vehicle and auto insurance at all times while employed with Visinet, Inc.
9. Collaborates with other professionals (team meetings, etc.) to develop a program that meets the specific needs of the client.
10. Ensures that Health Insurance Portability and Accountability Act (HIPAA) standards are maintained throughout the program.
11. Consistently reviews the Continuous Quality Improvement (CQI) plan and makes sure that all is being carried out according to established guidelines.
12. Returns pages in a timely manner (within twelve (12) hours for a regular pages and within one (1) hour of a 911 page) from 8:00 a.m. to 8:00 p.m. Monday-Friday and during regular availability on the weekends.
13. Maintains weekly contact with case managers/referral sources (leaving a voice mail on their voice mail if he/she is unavailable.
14. Ensures each client file contains appropriate consents and releases to ensure HIPAA compliance, including Consent to Treatment, Release of Information, Notice of Privacy Practices, and Client Rights and Responsibilities.
15. Collaborates with client(s) and client placements(s) in coordinating a consistent weekly schedule between child(ren) and caretaker(s) identified on referral.
16. Assists the client/family in obtaining the goals identified by the referral source and illustrated on the treatment plan.
ESSENTIAL FUNCTIONS OF THE POSITION AND PERFORMANCE STANDARDS
1. Contact supervisor (from 8:00 a.m. to 5:00 p.m. Monday-Friday) or the on-call supervisor (on weekends/holidays/after hours) in the event of a crisis regarding a client or individual associated with the client (regardless of the outcome a supervisor is always required to be notified).
2. Consult the COO/CEO and /or Clinical Director on all potential liability issues.
3. Strictly adhere to the proper chain of command as illustrates in the EM.
4. Communicate PTO requests to assigned supervisor as necessary while ensuring appropriate coverage of all cases.
5. Problem solves crisis issues in an effective and efficient manner; staffs all clinical issues with supervisor immediately upon report of a crisis situation.
6. Support and promote programs within Visinet, Inc. and mediate conflicts with community contacts to preserve the reputation and perception of consumers and stakeholders.
7. Attend weekly meetings with supervisor to provide case updates, address changes and turn in completed documentation.
8. Attend team meetings regarding clientele only when requested by the therapist/referral sources an approved by Visinet, Inc. supervisor.
9. Complete 12 hours of Mandt training.
10. Complete CPR training or recertification for CPR training.
11. Attend court hearings when subpoenaed.
12. Attend twelve (12) ongoing education/in-service hours per year or one (1) per month if employment begins beyond the start of the calendar year.
13. Complete service training and show competence in areas identified in the State Regulations (Chapter 32 of the Nebraska Medicaid handbook).
14. Responsible for checking mail weekly.
15. Complete all detailed documentation within forty-eight (48) hours of conducting a session.
16. Complete time sheets, provider reporting forms and progress reports for each case and turn in on the respective billing days as indicated in the Employee Manual.
17. Complete pertinent incident reports and provide to the supervisor within twenty-four (24) hours of the incident.
18. Complete requested re authorization forms by the Friday prior to the week the current authorization expires.
19. Complete all necessary assessments and outcome measures, as dictated by the program within the time frames outlined in the Program Specific Manual.
20. Contact all parties within twenty-four (24) hours after receiving a new referral.
21. Transport children (only) identified on a respective referral from their placement(s) to the session location.
22. Conduct sessions that integrate activities and interventions consistent with the identified treatment plan and the Axis I diagnosis.
23. Gather necessary information regarding client’s current behavior(s) to develop baseline data.
24. Remain within the guidelines of the referral at all times, as well as, within the required authorization period and number of hours allocated to the client.
25. Implement and utilize resources during sessions which relate to the identified goals on the treatment plan.
26. Other duties (not limited to the above) as determined by Visinet, Inc. Administration.

External: Continual contact with families, CFSS, Service Coordinators, and other stakeholders
involved with the child/youth and family.
JOB SPECIFICATIONS AND WORK REQUIREMENTS
1. Education
a. Required: Associates degree in social work or related field.
b. Desired: Bachelors degree in social work or related field.
